First Visit

Your first appointment with at our office usually consists of an examination to determine your dental problems and review of your treatment options. Sometimes simple treatment can be done or started the same day as the consultation. However, a complex medical history or comprehensive treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Any x-rays taken by a previous dentist, if applicable. (Call us to give you a release of information form)
  • A list of medications you are presently taking and any medical conditions .
  • If you have dental insurance, bring the necessary information like plan name and policy numbers . This will save you time and allow us to quickly process any claims usually electronically.

Please alert the office if you have any medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, epilepsy, etc.) or if you are currently taking any medication (i.e., heart or blood pressure medications, insulin, aspirin, blood thinners, etc.).

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or legal guardian.

X-Rays

If your previous dentist or physician has taken x-rays, you may request that they forward them to our office. Ask us to send you a release of information form for you to sign that authorizes the previous office to send your xrays to us. If there is not enough time, please pick them up and bring them to our office. If additional x-rays are necessary, digital xrays can be taken at our office.
